Usually, this acronym is texted over the phone or a few other messaging platform (on facebook or instagram), but it can occasionally be used in spoken speech too, normally by youths who tend to text usually or are trying to be amusing. When spoken, it is noticable T-B-H (ti-bi-aitch).
Additionally note that there is not actually a typical capitalization or punctuation standard for TBH and comparable phrases, because texted speech itself is not extremely standard.
Young people have the tendency to write it out lowercase (either tbh or Tbh), with or without a comma following it. Those who are less tech-savvy could spell it out in all caps (TBH).
